Release of the Executive Summary of the Combined Civilian Casualty Team Assessment Report on the Airstrike on Kunduz Hospital

MONS, Belgium - Gen. Philip M. Breedlove, the Supreme Allied Commander Europe, released the Executive Summary of the Resolute Support and Afghan Combined Civilian Casualty Assessment Team (CCAT) report on the 3rd of October 2015 airstrike by U.S. forces, which mistakenly targeted a Médecins Sans Frontières (MSF) hospital in Kunduz City.
The Resolute Support CCAT, led by U.S. Army Brig. Gen. Richard C. Kim and Afghan Col. Mohammad Kabir, conducted an investigation in parallel with the U.S. investigation, which was briefed by General John F. Campbell, Commander, in his U.S. capacity, at Resolute Support Headquarters, Kabul, Afghanistan on 25 November 2015.

"This was a tragic mistake and I share the sadness felt by the people of Afghanistan and Médecins Sans Frontières over this loss of innocent life," said Gen. Breedlove. "I have read the assessment provided by the Combined Civilian Casualty Assessment Team, which supports the findings of the U.S. investigation into what they termed a tragic, but avoidable accident caused primarily by human error by their forces. The lessons from this mistake must and will be learned and appropriate corrective actions taken to minimise the chance of it ever happening again.”
The CCAT figure of 31 deaths came from early reporting by MSF to the CCAT investigators. The U.S. national investigation's figure of 30 deaths came from the Initial MSF Internal Review. We are continuing to work with MSF to identify the injured and the families of those killed and reconcile early reporting from their initial internal review.
